What the label covers
The label is a shorthand for the conditions a piece of gear was designed around: rain, wind, cool nights, maybe a frost, no sustained snow load and no serious cold. It's the default for almost everything sold for backpacking, because it's where almost all backpacking happens.
What it means item by item
It means different things on different items. On a tent, three-season means a mesh-heavy inner for ventilation and a pole structure that isn't built to hold snow, which is why a four-season tent has more poles and less mesh. On a sleeping bag it's roughly a rating in the 20 to 32 F band. On a pad it's an R-value somewhere between 3 and 4.5.
Seasons are not the same everywhere
The honest caveat is that seasons aren't the same everywhere. Three-season in the Sierra in August and three-season in the Scottish Highlands in October are different propositions, and altitude moves the line further than the calendar does. Treat the label as a starting point and read the actual specification, since that's the part that doesn't depend on where the marketing team lives.
