How does a satellite messenger work?
It talks to a satellite constellation instead of a cell tower. Iridium, which most consumer units use, has satellites in low orbit covering the entire planet including the poles, so coverage is genuinely global rather than merely wide. Globalstar, used by some other devices, covers most of the populated world and leaves real gaps.
The device needs a clear view of the sky, which is the practical limitation people underestimate. In a narrow slot canyon or under dense conifer cover a message can take many minutes to get out, or fail entirely. Sending from an open ridge takes seconds.
What happens when you press SOS?
The SOS goes to a private emergency coordination center staffed around the clock, which contacts the search and rescue agency with jurisdiction where you are. On a two-way device you can then message back and forth with them, which matters enormously: they learn whether you have a broken ankle or a cardiac event, and can send the right response rather than guessing.
That two-way conversation is the main functional difference from a personal locator beacon, which broadcasts a distress signal and your position and then tells you nothing. A beacon is simpler and needs no subscription; a messenger tells the responders what they are coming to.
Is a subscription required?
For a satellite messenger, effectively yes. The hardware is inert without an active plan, and plans are usually sold monthly with a cheaper annual commitment or a more expensive freedom to suspend in the off season. Budget for it as an ongoing cost rather than a one-off purchase.
A personal locator beacon is the opposite: buy it once, register it with the national authority for free, and replace the battery every five years or so. It costs nothing to keep, and does nothing except call for help.

Assuming it works from inside a pack
These need line of sight to a satellite. A device buried under a lid or worn against your back on the wrong side of your body will miss messages and delay an SOS. Clip it to a shoulder strap facing up, and step into the open before sending anything that matters.

Is a satellite messenger better than a phone with satellite texting?
Phone satellite features have closed much of the gap for emergency use, and are free on recent handsets. A dedicated messenger still wins on battery life, on tracking, and on working when the phone is dead, which is the scenario that tends to produce the emergency.
Will it work under tree cover?
Usually, eventually. Dense conifer canopy and canyon walls block the sky and messages queue until the device gets a window. Step into a clearing or onto a ridge if the message matters.
